            Christian Home Educators of Kentucky will be hosting the tenth annual home school graduation on May 10, 2008.  The graduation exercises will be held at the Shively Baptist Church in Louisville, KY  This graduation ceremony is open to all CHEK member families.

            By signing this form below, each member family, certify they understand that CHEK is merely acting as host for the graduation ceremony, and CHEK does not certify completion of any courses of study.  The parent/guardian/administrator of each home school is solely responsible for the educational accomplishments of their graduate.

            CHEK must have this form on file for every CHEK family graduate student, signed by the parent or guardian, for the graduating student to participate in the CHEK home school graduation ceremony.
